Pathway Clinic is a preferred provider for most Health Insurance Companies. Many Health Insurance Companies require prior authorization for behavioral health services. Please be sure to contact your health insurer prior to setting up an appointment so that you are aware of such requirements or any deductible, co-insurance, or co-pays that you may incur for your visit. The contact information for your insurer should be on the back of your insurance card under “Customer/Member Services.”

Unity Health Care and Physicians Plus Insurance Plans:

All of our providers are credentialed to provide covered services under both Unity Health Insurance and Physicians Plus Insurance. Both companies require prior authorization for all behavioral health services. To obtain an authorization, please call 1-800-683-2300.

Dean Health Plan:

All of our providers are also credentialed to provide covered services under Dean Health Plan. Dean no longer requires prior authorization for behavioral health services.

Employee Assistance Programs (EAP):

Employee Assistance Programs are cost effective, job based strategies for helping employees who are experiencing personal and mental health problems, which may interfere with their capacity to work effectively in their jobs. Pathway Clinic provides assessment, treatment, and referral services for those employees. Your employer can assist you with determining if your program contracts with our clinic.
